Position Overview:

The Patient Scheduler is responsible for handling phone inquiries, processing referrals, and coordinating with the Liaison and referral sources to confirm receipt and provide timely updates on referral status. This role includes reviewing health records, creating digital charts for new patients, verifying insurance coverage, and securing authorizations when needed. The Scheduler also calculates medication costs, obtains necessary approvals, and keeps the Liaison informed to support the referral evaluation process. Strong organizational and communication skills are essential in this fast-paced role.

Job Responsibilities:

  • Handles phone inquiries, checks for referrals, and processes them accordingly
  • Communicates with the Liaison and referral source, notifying them of having received the referral and
  • letting them know that an answer will be provided promptly on the acceptance or denial of such referral.
  • Reviews health records prior to accepting referral.
  • Creates a digital chart for the new patient.
  • Verify Primary Insurance coverage.
  • Obtains authorization (if applicable) by calling the appropriate insurance provider.
  • Costs out medications and obtains approval from the Administrator, if necessary.
  • Notifies the Liaison if the medication costs and insurance are approved so they can complete their evaluation of the referral.

Job Qualifications:

  • Must have a Bachelor of Science in Nursing, BSN
  • Minimum of one (1) year of experience with either: admissions in a Skilled Nursing Facility, hospital discharge, or case management required.
  • Knowledgeable of medical practices, terminology, and procedures, as well as laws, regulations, and guidelines that pertain to home health. Background in working with Medicare regulations in Home Care.
  • Proficient in the English language, both verbal and written
  • Proficient word processing, keyboarding, and data entry computer skills required.
  • Experience navigating clinical information system networks is required.
  • Able to relate positively, favorably, and cooperatively with others, including physicians, associates, patients, and personnel of outside agencies and organizations
